When submitting a claim, make sure to:
Review your leave request with your supervisor
Submit your leave request in Red Rover using the Paid Family Medical Leave option
Email the completed Minnesota Paid Leave Employee Checklist to the district office at PFML@lake-city.k12.mn.us
Submit claim with Madison National Life (NOTE: Use PFML@lake-city.k12.mn.us as the HR contact email) (Directions linked HERE)
