When submitting a claim, make sure to:
Review your leave request with your supervisor
Email the completed Minnesota Paid Leave Employee Checklist to the district office at kagre@lake-city.k12.mn.us
Review and submit your leave request with your building's Admin Assistant in Red Rover using the Paid Family Medical Leave option
Submit your claim with *Madison National Life (NOTE: Use kagre@lake-city.k12.mn.us as the HR contact email) (Directions linked HERE)
