Kindergarten Registration

Kindergarten Registration is ongoing. Your child must be 5 years old by July 31st, 2022.

Get started with enrolling your child by downloading/completing/signing these forms:

1. SIS-10W

2. Kindergarten Parent Information Sheet

We will also need the following items to enroll:

1. Original Birth Certificate

2. Proof of residence in the Kula area (bill with service address in parent/guardian names)

3. TB Clearance

4. Student Health Record Form
