ECE Registration Forms

Welcome to ECE at KIPP Colorado Schools!

We are so excited to have you as a part of our program.

Please review all forms below and print/return any fillable forms to your child's teacher on the first day of school. Please note, per CDPHE mandate, your child CANNOT be accepted into programming on the first day of school if you do not have a signed copy of the health form below or a general health appraisal form from your child's physician.

If you are unable to access a printer at home, printed packets of forms requiring completion can be found in envelopes outside the main entry doors at KSPE and KNE.

To fill out and return:

• General Health Appraisal Form (NOTE: your doctor can also provide this to you) English + Spanish- REQUIRES DOCTOR SIGNATURE, MUST HAVE TO START SCHOOL ON THE FIRST DAY
