Permission to tape form
Parent Permission to Tape
It has been recommended that your child would benefit from taping as a therapeutic intervention. The primary purpose of taping is to support weak muscles and to ensure proper joint alignment. Keeping proper joint alignment helps to prevent future orthopedic concerns and aids to keep your child in a proper upright posture. Having proper posture will better assist your child to perform gross and find motor skills with less difficulty.
Although the tape is latex –free, any allergies must first be ruled out. Your child will have test strip of tape on their back (so can not be easily removed) to test for any allergic reactions. If you child shows any allergic signs to the tape, then taping will not be performed. If not, then taping will proceed. The first week your child is taped you should keep an extra eye out for any redness or itching your child may experience. Let the therapist know immediately if this occurs.
The tape can stay on your child for up to two days. The tape is waterproof and can usually uphold a shower. If the tape starts to peel away within the first two days, you can rub the tape against the skin to re-adhere the tape. Please remove the tape slowly to ensure your child’s safety. The tape will be reapplied by the school therapist only. If you would like to learn how to tape you child at home or for the weekends, please schedule an appointment directly with me.
If you consent to the taping please sign below.
