Part 1 and Part 2 will need to be completed by the parent and students before visiting the nurse practitioner's office.
EL2_Form.pdf**Both the EL2 and EL3 forms are required by the state of Florida & MCSD to participate in extracurricular athletics programs. You will not be permitted to tryout without these forms completed by a doctor/nurse practitioner and signed by parents.
