Sports Physical

All forms submitted to the Coach BEFORE  you are permitted to try out 

Please make a COPY of your Sports Physical for your records 

KLS Athletic CONTRACT 2022-2023

This form (below) needs to be notarized. Also, a copy of your insurance information should accompany this form if you have medical coverage.

Part 1 and Part 2 will need to be completed by the parent and students before visiting the nurse practitioner's office. 

EL2_Form.pdf

**Both the EL2 and EL3 forms are required by the state of Florida & MCSD to participate in extracurricular athletics programs. You will not be permitted to tryout without these forms completed by a doctor/nurse practitioner and signed by parents.
