Care. Expertise. Integrity. Diligence.
“A life care plan is a dynamic document based upon published standards of practice, comprehensive assessment, data analysis, and research, which provides an organized, concise plan for current and future needs with associated costs for individuals who have experienced catastrophic injury or have chronic health care needs.”
(International Conference on Life Care Planning and the International Academy of Life Care Planners)
A life care planner is experienced in varied professional backgrounds. The life care planner is certified, licensed, and meets the legal state requirement to practice in their profession. The life care planner completes a post-graduate or specialty degree training specific to the areas applied to a life care plan.
The life care planner operates under a Governing Board, which provides oversight to protect the consumer; the Governing Board implements disciplinary actions when determined appropriate.
Most importantly, the life care planner agrees to be peer reviewed, adhere to a set of practice standards and ethical guidelines, and scrutinized or disciplined by the Governing Board.
We maintain an objective and diligent process to deliver a life care plan that clearly communicates the future medical needs of the individual following a catastrophic injury or chronic illness.
Our process:
Review of medical records
Interview with the client, family, or medical professionals
Review of living environment
Utilize evidence-based practice recommendations
Objective medical cost projections based on geography and available medical services
