Club Constitution Form (Put in Ms. Shannon's box)
Annual Update Form (Put in Ms. Shannon's box)
Advisor Acknowledgement (Give to Mrs. Memering)
Club Minutes (make a copy and save it to your drive)
Purchase Order Request Form (make a copy and save it to your drive)
Event Request Form (Check this spreadsheet to see approved Event Requests follow up with Ms. Shannon with any questions)
Fundraiser Contract (Have students sign a contract when you are selling anything that they take responsibility for - such as candy bars)
Cash Box Request Form (REQUIRES a minimum of 3 BUSINESS DAYS!)
Teacher Permission Form to Miss Class (Give to students to get teacher permission for pull-out days, field trips, etc.)
