WHAT SHOULD I BRING WHEN I AM ENROLLING MY CHILD?
To make the enrollment process as smooth as possible, please bring the following items with you when enrolling:
Proof of Residency
This can be your signed lease agreement, proof of purchase, or a current utility bill
Your photo ID
Your child's most current Immunization and Physical Record
Incoming 6th graders will need a NEW school physical and UPDATED shot record
Athletic physicals are separate from school physicals. Athletic physicals are required in order for students to try out for or participate in sports, but it does NOT count as a school physical.
Your child's Social Security Card
Your child's Birth Certificate
Any custody papers (if this applies to your situation)
A copy of your child's most recent report card, test scores, IEP and/or 504 plan (if your student has one)
The request for records form included in the enrollment packet linked above will be sent to your students' former school. Some of the information requested above may be available through your students' former school, however if you have copies at home, it speeds up the enrollment process if you bring them with you.
