1.Complete the First Four Pages of KHSAA packet (below) on Final Forms. Pages 5 and 6 (Preparticipation Physical Evaluation) Must be uploaded to Final Forms, in PDF or JPEG format. A Medical Insurance Card must also be uploaded (This can be all one file, to minimize uploads).

A. Pages 1-4 are completed by the parent and student. This must be completed each school year. They do not roll over.
B. Pages 5 & 6 are completed by a medical professional. The medical professional must sign and date the forms. These are valid for 12 months. If a copy of pages 5 & 6 have been completed within the last 12 months, you may upload those.

Each student will get this form at the beginning of the season from their coach. It will be turned in before competitions start.
