Kentucky Immunization Certificate-Parents get form from Health Care Provider or Health Department
Original Birth Certificate
Medical Examination-13/14 Physical Form
Copy of Child's Social Security Number
Free Lunch Form (When appropriate)
Proof of Residency - street address which may include utility bills to the legal guardian, rental/lease/purchase contracts, etc.
Transcript, report card, and/or other academic records from prior school(s)
Dental Screening Form-Due Jan 1st of the school year for ages 5 & 6 only.
Eye Examination Form-Due Jan 1st of the first school year that a 3-6 year old child is enrolled.

Brigance Testing
Within the first couple of weeks of school, your child will take the The Brigance Inventory of Early Development ii (IED-ii). While we use the word "test," it is really more of a one-on-one assessment with an adult to let us know where your child stands in 5 key areas of development in relation to his/her peers.
Those areas are:
Language Domain (receptive and expressive)
Motor Domain (gross motor and fine motor skills)
Academic-Cognitive (general/quantitative and pre-reading skills)
Daily Living Domain (self-help and prevocational)
Social-Emotional Domain (play skills and behavior and engagement/initiation skills)

Brigance Parent Survey: Please use the form below to tell us more about your child's social and self help skills.
You can bring both of the filled out forms to Kindergarten Registration or turn them in on the first day of school! Thank you for your help in gathering this information so that we can better support each student.
