This collection of resources is designed to empower adults and their families to prepare for a good end of life experience. While 92% of Americans say it’s important to discuss their wishes for end-of-life care, only 32% have had such a conversation, according to The Conversation Project National Survey (2018). This Libguide offers resources to help people begin thinking and talking about their wishes. By improving access to information, the goal is to reduce uncertainty and increase the likelihood that individuals' wishes are respected.
Resources are organized into categories, which you can access in the upper right. You'll find a mixture of helpful information and practical tools, ranging from conversation starter kits to advance directive forms.
