Athletic Information for 2025-26 School Year
Sports offered at Woodcreek Junior High are Football, Volleyball, Boys and Girls Cross Country, Boys and Girls Basketball, Boys and Girls Track, Boys and Girls Tennis, and Boys and Girls Soccer. Each page is separate under the "Sports" tab.

Required Forms
REQUIRED FOR Athletics/Sports Participation
INCOMING 7TH & 8TH GRADE
All required participation forms (Annual Physical, Medical History Form, KatyISD Consent to Treat, UIL Forms Signature Page, Handbook Acknowledgement Form) must be submitted before a student participates in any try-out, practice, athletic class, open gym, open weight room, summer program (SAC Camp), athletic competition or travels with a KISD athletic team for any purpose.
The only required form that cannot be completed electronically is the annual physical and medical history form. Parents will be able to print a physical and medical history form to take to your physician or school sponsored physical event.
Physical Information
Must be completed by a doctor after May 1, 2025 (physicals with a date before May 1, 2025 will not be accepted for the 24-25 school year).
Physicals can be printed to take to your doctor HERE.
Physicals must be uploaded to Rank One Sport.
