REQUIRED FORMS FOR ALL POTENTIAL ATHLETES
INCOMING 7TH & 8TH GRADE
All required participation forms (Annual Physical, Medical History Form, KatyISD Consent to Treat, UIL Forms Signature Page, Handbook Acknowledgement Form) must be submitted before a student participates in any try-out, practice, athletic class, open gym, open weight room, summer program (SAC Camp), athletic competition or travels with a KISD athletic team for any purpose.
The only required form that cannot be completed electronically is the annual physical and medical history form. Parents will be able to print a physical and medical history form to take to your physician or school sponsored physical event.
Physical Information
Must be completed by a doctor after May 1, 2023 (physicals with a date before May 1, 2023 will not be accepted for the 23-24 school year).

Physicals must be uploaded to Rank One Sport.
