Before presenting research at both SMSEF and ISEF, forms must be filled out to ensure compliance with ISEF rules that might apply to projects. When registering for the fair, these forms (3 at minimum) MUST be filled out and approved to compete at SMSEF. Although not all forms are required when presenting at SMSEF, if a form is relevant to the research, it MUST be filled out. (If the research is being presented as a team, one team member must be elected as the "Team Leader" who will be responsible for filling out a majority of the forms.)

Form Deadlines:
If your project requires prior SRC approval (Forms present in the "Additional Forms" tab), forms must be submitted to rebecca.joyce@kamsc.org by December 4th. (Examples of projects that require pre-approval: humans, vertebrate animals, bodily fluids, bacteria)
All other forms must be submitted to rebecca.joyce@kamsc.org by February 16th
Forms Required for All Research Projects
This form serves as a "Overarching" form as it helps ensure the researches as well as an adult sponsors has read all of the governing rules. The form also helps the researcher understand what forms are needed to present thier research
This form helps collect metadata related to the research(s) including decleration of other team members in the project. Alongisde this form, a research plan must be sumbitted for approval (Instructions on page 2 of the form)
Form B serves as a final confirmation that the student as well as their parents will abide by the rules of the fair. After completion of the form, the research will get approved from the International Review Committee (IRB) and will sign the same form
Form Required for Most Research Projects
This form is required for projects that involved the use of a mentor. Almost all projects that have competed at SWMSEF in the previous years have had the help of a mentor and worked in a research setting, thus been required to present this form
If your projects involves procedures involving humans or animals, it is likely the research will require additional International Review Board (IRB) review. To understand what additional forms must be needed, take the survey attached to the link above. If your research is a continuation of previous research project, Form 7 must also be filled out.
