Please fill the form above for each player who is attending the 605 Hogs Camp. Cost is $60 for player. Checks can be written to Shawn Flanagan or Venmo: Venmo is @coach_shawnflanagan last 4 of phone number is 7850. Mail checks to 304 East 2nd Avenue Lennox SD 57039. Payment/registration is due June 7th for T-Shirt Orders. Registrations after June 7th will not be guaranteed a shirt Make a note for what grade and player the payment is for (Last, First) name. Please fill one out for each player attending.  A copy of the form will be sent to you via your email. Please input the email that you use.   Camp cost is $60 if payment and registration is received before May 7th. If after camp cost will increase to $75. If Camp is canceled due to rain/weather half of registration will be refunded to cover administration cost and shirts/ food that has to be pre ordered.
