Returning (your child or sibling of a child has participated in sports at Kailua High School in recent years) Parents and Student Athletes must follow these instructions to be compliant with the Athletic Department's requirements. All forms require a birth parent/legal guardian to fill out and sign.

1. The birth parent or legal guardian attends a mandatory Parent Meeting. All Sports meeting for SY 2025-26 is scheduled for Wednesday, August 7, 2025. At this meeting, you will fill out the Parent Verification - this is required for each student athlete - submitted once while attending Kailua High School - good for all four years unless rescinded or released from Kailua High School.

2. Watch all required videos and review the DOE Parent and Student-Athlete Handbook, fill out the Athletic Program Verification Form, and submit it to the Athletic Health Care Trainers. Hard copy forms can be picked up at the Training room.

3. Complete an annual Sports Physical, fill out, and submit the following documents to the Athletic Health Care Trainers: 

   • Concussion Management Program and Physical Examination for Athlete Forms (once a year)

   • Student Participation and Parent/Legal Guardian Consent, Release, and Assumption of Risk Form (one per sport)

4. Fill out the KHS Parent Verification Form and submit to the Athletic Director. Required for each student athlete - this will need to be submitted only once while attending Kailua High School - good for all four years unless rescinded or released from Kailua High School.

All Parents and Student Athletes must watch these videos each year to be compliant with the Athletic Department's requirements.
