REGISTRATION CHECKLIST

It is best to submit as many of the registration documents all at once so they are bundled together. However in some cases, like scheduling a doctor visit may take some time, therefore it is okay if these come in at a later time. Our office will follow up with you to make sure all items are received prior to the start of school.

1. Proof of Current Residence - NEW ADDRESS VERIFICATION POLICY:

2 different utility bills (e.g. cable, water, electric, gas, land-line service);
- 1. The bills should be in the legal guardian's name
  2. Dated within the last 60 days
  3. Bills show that they are being sent to the home

An example of an acceptable proof of residence consists of a current utility bill showing the parent’s name, address, and recent date.)

If residing in military housing, a signed lease agreement or housing acceptance letter (provisional) is sufficient;
