Guide to LHS Sport Clearance - PLEASE READ

Any prospective student-athlete will need the following documents in order to participate in Lahainaluna High School sports. These forms must be turned in by the student directly to the athletic trainers in the athletic training room (ATR). For any questions please contact your sport’s coach or the athletic trainers.

Part 1) Health Forms -> Athletic Trainers

Please turn these forms directly to the LHS Athletic Trainers who are located in the athletic training room (ATR). Forms turned into the main office will not be seen or count towards participation. Incomplete forms will be returned to the student and must be completed properly and turned back in for student to be cleared for sports participation.


  1. A HIDOE Physical Examination for Athletes (form RS 13-1286) signed by a qualified clinician. Only individuals with an MD, DO, APRN, or PA are certified to sign the document. This physical must have the original signature of the clinician, and cannot be a photocopy. The physical expires yearly and the student-athlete will need to obtain a new form each year. An adult student (18+ years old) may sign in place of a parent or guardian.

    • This form includes the Concussion Management Program Acknowledgement that must be signed by a parent or guardian. An adult student (18+ years old) may sign in place of a parent or guardian.

  1. A HIDOE Consent, Release, and Assumption of Risk (form RS 13-1288) signed by the student-athlete’s parent or guardian. An adult student (18+ years old) may sign in place of a parent or guardian.

Part 2) Clearance Forms -> Athletic Director

Please turn these forms into your coach or the athletic director. All videos much be watched and paperwork done prior to sports participation

  1. Parents/Guardians of student-athletes will need to attend a Parent Meeting for Sports held in the fall, winter, and spring. They will also need to view the “OC16 Risk Management” video and the “HCAMP Know Your Role” video. Read over the Parent and Student-Athlete Handbook with your student-athlete prior to participation. After completing these items fill out and submit the Complete of Requirements Form which will be given out during the sports parents meeting.

Transfer students wishing to participate in athletics:

For those students who transferred to Lahainaluna High School, the Maui Interscholastic League has a transfer policy.

Please notify the athletic director, Jon Conrad if you have any questions or to see if your child is eligible for participation.
