If you would like to request to use any of the equipment that you saw on the inventory page then you must fill out this form below to schedule a time that you will be able to come to the tech department room and pick up the piece of equipment. Once you go there to get it you MUST fill out a technology check-in/out form that you will have to fill out at the department room that will be provided by the DPUST. You will also have to fill out a Google form that consists of the same thing on the paper form so that we can transfer the information into our inventory database to keep track of each piece of equipment. The Google form will be on this page so you can easily find it and fill it out after you get your device.
Above are two google calendar pages that you will be able to access and set up a time to either check a device out or to check a device in. We did this so we can track who is coming in so we can already have the device set up and ready for you. All you would have to do is complete the checkout form on paper and also the one online.

Answer all the questions below. Make sure to put which DATE you are checking a device OUT or IN, you do not have to answer both but make sure that one of them has the date, Mahalo!
