ACE
Acute Concussion Evaluation and Care Plan
JP has procedures in place for students diagnosed with a concussion.
Your primary point of contact is the School Nurse and/or the Athletic Trainer
Concussions CANNOT be diagnosed by x-ray, CT or MRI
Concussion diagnosis is symptom based.
Students suspected of having a concussion should be evaluated immediately by a medical provider using the ACE Concussion form.
It is imperative that page 2 (the Accommodations page) of the ACE form be completed by your student's medical provider so that the student can receive the necessary accommodations at school to aid in their recovery.
The Accommodations page of the ACE concussion form below needs to be returned to the school nurse as soon as possible.
The Accommodations Page provides the school nurse and athletic trainer (if the student is involved in athletics at JP) with instructions for your student's concussion care from the medical provider.
The school nurse (and athletic trainer) will involve your student's guidance counselor and teachers when the accommodations are received.
