New-Student Enrollment Instructions
Enroll a NEW STUDENT at WJMS in Three Steps:
Step 1 - Find/Confirm Your Student's School
Visit our Bus Stops & School Boundaries page to locate the boundary school for your address.
IMPORTANT: If you find that your home address is not located within the boundaries of West Jordan Middle School, you MUST first complete Jordan District's permit Open Enrollment/School Choice request form and await approval. If approved, proceed with Steps 2 and 3. If not approved, you will need to enroll at your boundary school.
Step 2 - Complete Online Enrollment
* If your child has never attended school in Jordan District, complete the New Student Enrollment: Account Request form. A link will be emailed to you along with a temporary login and password to complete the Enrollment Application.
* If your student has attended Jordan District, complete the Enrollment Application by using your existing parent Skyward login and password.
Step 3 - Gather Documents and Enroll at School
Please be preapred to bring necessary documentation for your studen't enrollment.
Items include:
* 2 Forms of Proof of Residency (mortgage or rental/lease agreement PLUS any of the following: payroll stub, bank statement, utility bill, post office address change confirmation, valid driver's license, current vehicle registration, valid passport, current W2 form, property tax bill, letter from approved government agency such as assisted housing, food stamps, or unemployment payments)
* Parent/legal custodial guardian's picture identification (driver's license, state agency picture ID, or passport)
* Child's original or certified birth certificate
* Immunization record
* Unofficial transcript or withdrawal form from previous school (if transferring to WJMS during the current school year)
