NEW AND CONTINUING CLUBS MUST:
COMPLETE A CHARTER FORM AND A BUDGET FORM (beginning of year) BY Oct. 10th, 2025/ Jan.30th, 2026
(Sports teams-separate form- due within 2 weeks of season starting)
ONCE APPROVED, CLUBS CAN THEN SUBMIT BUDGET/PO REQUESTS AND FUNDRAISER AND OR EVENT REQUESTS/PARTICIPATE IN FOODS DAY etc
IMPORTANT FORMS:
k12 Club Information- Charter form / Constitution template (2025= ALL CLUBS should revise or turn in new!)
Beginning of year budget form
End of year budget form (linked in spring)
Meeting Minutes templateย
Each club, class, team will have a folder in JHS ASB shared drive where minutes etc must be keptย
All clubs, classes, teams will create minutes on a google doc for each meeting for the entire year (link Must be attached with any Budget/PO or Fundraising/Event request)
K12 forms:
Budget Request/PO form : attach PDF of meeting minutes (approval/confirmation of expenditure of Club Funds BEFORE you SPEND ANY MONEY FOR ANYTHING)
Reimbursement/ASB check request => funds can only be reimbursed if the PO request was done/approved and pic of ORIGINAL receipts are attached
OTHER IMPORTANT FORMS:
Event Request form -All events that require a space, room etc on campus at anytime of day must complete the PAPER request form with all signatures complete and submitted to Ms. Bibiano for approval on CIVIC PERMITS
Change/Cash box request form: print out and turn into Ms. Santos 2 weeks ahead of date needed- please make sure the adviser is aware and can get the box BEFORE the day of.
