The New Horizons Transition Program is primarily focused on preparing our students (and their families) for life after they graduate from Churchill Park. As a family, if you're interested in your child doing supported employment, we will do all we can to help you pursue that as your child transitions out of our school and into their New Horizon. Howbeit, the majority of our families are genuinely focused on finding a day program that best fits their child's wants and needs and a place their child will be loved and greatly cared for -- like here at Churchill Park! So, that's where we come in. Over the last couple of years, of each student's life at Churchill Park, we will work closely with each of our families to find the right program, and supports, for their child. Every family is looking for something different, and we're here to take all those wants, needs, and desires into account as we help you navigate this process.
The city of Louisville has dozens and dozens of day programs and agencies -- which is great! But it can be overwhelming for our families to navigate alone. And thus, we, The New Horizons teachers, have already toured and vetted most, if not all, day programs in the area, and we've built relationships with the staff/directors in these programs. We have also carefully crafted profiles, of each program we've visited, as a way for us to better select the programs our families are looking for without having our families go through the motions of seeing every single place -- that can be daunting. As we sit down and conference with each family, we will gain insight into what you are looking for as a day program, and we will then share the programs that best meet those needs and wants for your kiddo. Then we will set up tours of your top picks and meetings with their directors. And lastly, we will connect with your kiddos case-manager since they are the ones who submit day program paperwork. We often work closely with our student's case-managers.
Not only do we help our families seek out day programs, but we help families with various supports such as future residential planning, respite care, CLS, CLS care-plan creation, obtaining waivers (CSB, Michelle P., SCL), SSI, getting adapted quipment, getting a speech device for home, etc. And at the end of the day, we want our students (and their families) to genuinely love and enjoy their next move in life post-Churchill Park. This process takes a village, and we are joyfully a part of your village!
Crusader For Life Program
Churchill Park School is not only committed to being a resource to our families while your child is a student at Churchill Park, but we are committed to being a resource to our families forever. Hence the name, "Crusader For Life" Program. The Churchill Park Transition Support Teachers can (and will) always be a point person for you an your family if you ever need help with anything transition related (e.g. you want a new day program, want to consider having your child pursue a residential setting, want respite care, SSI, changing waivers, and/or help with anything else related to transitions that may take place. So please, take advantage of this program if you ever need it!

Thank you for your dedication to your students' education always, especially during this uncertain time. Please see any of the links below to get information regarding your medicaid waivers. The Cabinet for Heath and Family Services is working hard to provide resources to families in a variety of ways. As always, the 'New Horizons Transition Program' encourages you to keep up your case managers and brokers to keep your waiver active. Additionally, there are links to resources should you need them.
