Sport Physical Packet
If your student is interested in participating in ANY sport at Highland they must have a CURRENT athletic physical on file. With the COVID pandemic these are becoming more crucial in that ALL parts are completed on ALL pages or the student will be unable to participate.
**Please pay attention to the following pages:
Pages 3 & 4 emergency contact, parent & student signature for liability wavier
Page 10 must have a notary at the very bottom
Page 7 must still be signed even if you check "NO" for all
Double check page 2 to make sure you completed all parts
