JCPS has now moved to a digital format. All REQUIRED school forms can be found on the Parent Place website (not to be confused with Parent Portal), in the Complete Forms option. Please allow 15 minutes to complete per child.

Car Rider Tags
All car riders will receive new car tags. Please email Mary.Barnes@jefferson.kyschools.us if you were not able to get a new car tag at registration.
VILS Device End User Agreement (EUA)
All students must have a guardian complete this online End User Agreement form with Digital Promise/Verizon Wireless in order to receive their Chromebook, on loan, for the school year. There are 10 sections to this form and a confirmation email will be sent to you upon completion of the form.
