· Preventative Healthcare Examination Form AKA 6TH Grade Entry Exam Form- (Healthcare Provider to Complete)
· Immunization Certificate and Requirements- (Healthcare Providers will need to provide an updated certificate after 11 year vaccines)
· Nurse Office Consent- required for school nurse to administer ANY medication, including cough drops, Tylenol, etc.- (Parent to Complete)
· School Health Plans- required for ANY health condition(s)- (Healthcare Provider to Complete)
· Permission Form for Prescribed or Over the Counter Medication- required for students needing to take medication while at school (this means they are unable to take the medication prior to coming to school or they take medication more than once per day)- (Healthcare Provider to Complete)
