You can obtain/have your transcript sent by one of the following ways: 

1. Fill this out and email it to cpoat@jackson.k12.mo.us along with a copy of your picture ID.

2. Print this, fill it out, and bring it along with a picture ID to the counseling office. 

3. Come to the counseling office and get a physical copy, fill it out, and bring it back to the office along with a copy of your picture ID.
