Name: Your first and last name must match your visa, paychecks, and ID.
Social Security Number: Your Social Security Number (SSN) must match your Social Security card and the Form W-4 you submitted to your employers.
U.S. Address: This is where you lived during your Summer Work Travel program. If you moved, you will need to provide additional addresses.
Home Address: This is your address in your home (foreign) country.
ID: To verify your identity, you will need to upload a photo of your ID or Passport, and a selfie. If we cannot verify your identity this way, we may ask you to upload a brief selfie video.
