Athletic Clearance
Families of New and Returning Vaquero Athletes :
Please read below as this pertains to everyone interested in athletics at Irvine High School.
There have been some changes made to the athletic clearance forms and we want to be sure you all have and are using the newest forms. Please see the highlighted changes.
The process of clearing a student for athletics involves setting up a profile on homecampus.com, answering questions, and uploading forms.
Clearance requires the upload of 4 forms (physical form, statement of consent & signature confirmation form, insurance card front & back) :
Physical Form – top section (Health History) is to be filled out, signed, and dated by the parent/guardian. If you answer “yes” to any questions, you must explain specifically in the box under #25. The bottom section (Physicians Section) is to be filled out, signed, dated, and stamped by a physician (physician must check off the level of activity, and forms must be signed, dated, and stamped.
We do have a new Physical Form. It does look very similar to the old form but it has been updated. The date revised on the bottom left should be 4/18/22. This is the only Physical Form Accepted. IUSD Physical Form.
The only exception to this is athletes with Kaiser insurance. For those with Kaiser, you must upload the 1 page physical from your Kaiser physician but must still upload the top section of our physical form (health history section) which is filled out by the parent, signed, and dated (so Kaiser athletes will have 5 forms total) (*Again, If you answer yes to any questions, please be sure to explain under question #25)
Please do not upload any medical documents aside from the IUSD Physical Form and (if applicable) the 1-page Kaiser physical form. Please do not upload vaccines and immunizations.
Statement Of Consent & Signature Confirmation – We do have a new Statement of Consent & Signature Confirmation. This form must be hand-signed (wet signature) and dated by both athlete and parent. Statement of Consent & Signature Confirmation Form. This is the only form we accept.
Insurance Card – Please upload a picture of the front and a picture of the back of your students actual insurance card. Screenshots off insurance websites will not be accepted.
Please be sure all uploaded forms are legible.
Please be sure to write down the username, email, and password you use. All changes should be made to this profile only. We ask that you do not create any duplicate profiles. One profile per student.
It is no longer necessary to email when you have finished setting up your students clearance. The school will be notified and will check over the profile shortly thereafter.
***Please check spam folder for email from school regarding clearance/denial information ***
If you have any questions, please go to the athletic clearance page on the Irvine High School Website. Clearance Information
Thank you all for your patience and understanding. We are looking forward to a great 2024-2025 year.
