If you need to reschedule a  "telehealth" session/check-in session, please fill out the quick Google Form form below, or if you would like to request a (non-emergency*) meeting outside of your weekly "telehealth" session.

Please allow 48-72 hours for a response, with the exception of summer, holiday breaks, weekends, and non-office workdays.

*If you are having a life-threatening emergency, and you or someone you know requires immediate assistance, please call 911 or go to your nearest hospital emergency room.
