NTACT:C provides information, tools, and supports to assist multiple stakeholders in delivering effective services and instruction for secondary students and out of school youth with disabilities.

The Pennsylvania Community on Transition is a group comprised of various stakeholders from across Pennsylvania who work collaboratively to ensure appropriate transition outcomes for Pennsylvania youth and young adults with disabilities. The team consists of representatives from the State Departments of Education, Health, Labor and Industry, and Human Services; Various Youth-Serving Agencies; Young Adults; Parent Organizations; Advocates; Higher Education; and Employers.

The Commonwealth Technical Institute (CTI) at the Hiram G. Andrews Center (HGAC) prepares Pennsylvanians with disabilities for the careers they want with individualized, affordable education and training to contribute to our workforce in a meaningful way.

Their specialized staff also provides supportive services to prepare Pennsylvanians with disabilities for competitive employment.

The primary focus of Transition Discoveries is to empower communities to share leadership and collaboration to plan for life after high school with young people with disabilities, their families, and stakeholders.

The mission of the Pennsylvania Youth Leadership Network is to coordinate a network led and driven by inclusive organizations of youth and young adults with and without disabilities across Pennsylvania. 

EducationPlanner is your one-stop career and college planning website. EducationPlanner provides practical and easy-to-understand advice to help prepare students for the important decisions they will face in the future.

The I’m Determined project, a state-directed project funded by the Virginia Department of Education, focuses on providing direct instruction, models, and opportunities to practice skills associated with self-determined behavior. This project facilitates youth, especially those with disabilities to undertake a measure of control in their lives, helping to set and steer the course rather than remaining the silent passenger.

Deafverse is a choose-your-own-adventure online game created just for deaf teenagers, where they can learn important skills for success in high school and beyond.
