To participate in your school science fair you must complete specific forms. These forms can be overwhelming but this site should help carefully walk you through the process!
STEPS to Follow:
Know when your child's school science fair is scheduled and who your contact is for specific school science fair questions.
Help your child determine a topic and make sure that the topic is suitable to the rules! General Rules
Please remember : No Elementary students can do projects involving culturing micro-organisms (bacteria, mold, fungus, etc.) in any location (school, home, or regulated institution) no exceptions No Junior (Middle) or Senior students can do projects involving culturing micro- organisms at home no exceptions. These two reasons are why projects are not allowed to compete in most circumstances!
Prepare a Research Plan! Required for all Projects! This happens before you begin experimentation! (More information Below)
Complete required forms as you go! DO NOT WAIT UNTIL END. (More information below)
Be Careful to know if additional forms might be needed! Some projects require additional forms besides the ones explained here! Use the Science Rules Wizard prior to project start to find out!
Complete a Research Plan REQUIRED 1 Per Project
Your research plan is written prior to experimentation!
Remember this is not a report or a research paper, it is a plan!
Must include:
Question or problem being addressed
Hypothesis/Engineering Goals
Detailed description of methods or procedures
Please note that data is not to be included
Bibliography – minimal 3 for Elementary and 5 for Junior and Senior

Form 1 Adult Sponsor Checklist REQUIRED 1 Per Project
Completed prior to experimentation!
Complete the Student Name and Project Title (Should be the same title you put on your project board)
Everyone must completed questions 1-5 checking appropriate boxes.
Additional Forms section must be reviewed and determine if any additional forms other that what is listed as required for everyone might be needed.
Adult sponsor (usually this is the parent) must sign this form and date it prior to experimentation start date!

Form 1A Student Checklist Required 1 Per Project
Information form to include contact information (email addresses will be used to contact you about the status of your project), name of project, adult sponsor name and address where research took place.
Please fill out ALL information
Pay Attention to Experimentation Dates! These should read so they start after you have completed your research plan and form 1.
Explain where the experiment will be conducted, certain experiments are not allowed at home
HERE is where you will attach your RESEARCH PLAN! (You will see additional directions on this form on how to complete a Research Plan)

Form 1B Approval Form Required 1 Form Per Team Member
Form 1B Required Per Participant
Dates on this form should read prior to experimentation start!
Complete box 1 prior to experimentation, student and parent/guardian must sign!
ON BOX 2 and Box 3 STOP unless you are using human participants, vertebrate animals, hazardous biological agents OR an outside research facility! If any of these apply to you, send this form to your school science coordinator, they will send to khinson@iss.k12.nc.us for signature.
ON BOX 3 do not complete this box, it is only used at regional and state levels.

ABSTRACT Form Required 1 Per Project
THIS Form happens AFTER experimentation!
What is an Abstract? A summary of experimentation. This form also serves as a checklist for any additional forms that may be needed.
Maximum of 250 words. An abstract is written after experimentation and includes a few sentences of each area below:
a) purpose of the experiment
b) procedure
c) data collected
d) conclusions

Risk Assessment Form Required 1 Per Project
This form is required, even if it seems silly and does not apply, write N/A on each question. Every project should answer every question and signed by adult as well as dated prior to experimentation!
