Eligibility Forms

The following forms are required by the Minnesota State High School League(MSHSL) in order to participate in high school activities. In addition to these forms, a fee is required by the St. James High School activities office, which will need to be paid prior to participation. Forms and fees should be taken care of through the high school activities office.
