Developmental Disability Services provided through Social Services determines eligibility for services, and assesses the needs for adults and children with developmental disabilities. The County identifies service and support options based on need and available funding. County staff provides information, advocacy and consultation.

Eligibility

  • For children age birth to 5 years - To be eligible, the child must be determined to have a substantial developmental delay by your school and/or medical professional.
  • For individuals age 5 and older - To be eligible, the person must have an IQ and adaptive scores under 70. A person may also be eligible if they have a related condition such as cerebral palsy, autism or a seizure disorder, and they must have limitations in areas such as self-care, communication, social skills, learning, mobility, self-direction and independent living.
    • The IQ assessment must be conducted by a licensed psychologist or school psychologist. Dakota County may be able to pay for the assessment if there is no other funding source.

  • You will take the MnCHOICES Assessment to help determine your need, and you will receive information on how to get a social worker who specializes in Developmental Disability Services. Even if you are not developmentally disabled, other services may fit your needs.

The MnCHOICES Assessment will identify the support and services each individual needs. Some of the options include:

  • In- and out-of-home respite care
  • In-home staff supports
  • Behavioral interventions
  • Community integration activities
  • Home or vehicle modifications
  • Therapies, special diets, or equipment not covered by insurance or other sources
  • Daily living skills
  • Employment supports
  • Housing supports
  • Recreational opportunities (see the Recreational Directory)
  • Personal care attendant
  • Self-directed options
  • Nursing
