A well-being screener is a brief check-in that helps schools notice if a student may be struggling with things like anxiety, depression, or thoughts of self-harm. It also helps identify a student’s strengths, such as positive school experiences and their ability to cope with challenges.
Parents who would like to see the questions before their child completes the screener are welcome to do so. Because the questions are protected by copyright, they cannot be posted online or sent by email. However, parents may schedule an appointment with Tim Marriott (see contact information below) to review the questions in person.

Students in grades 3-12, with parental consent, who are currently enrolled in the Iron County School District.
Well-being screenings are similar to other routine screenings that schools provide, such as hearing, vision exams, and reading screeners. Research shows that when students have higher levels of personal well-being, they have better academic outcomes.
Upon completing the parental consent form, parents will be sent an email with a link to an online well-being screener. It will be the parents responsibility to provide the link to their student. Younger students will likely need help from a parent to complete the well-being screener. If student's are at-risk in the area of anxiety, depression or self-harm, The student and parent(s) will meet with a ICSD mental health professional to discuss the results of the screener and identify any support needed to help the student be successful. A link with the student's results will be emailed to a parent.
The well-being screener is optional and provided as a service to families. Parental consent and participation is mandatory! No Exceptions!
The well-being screener will be administered March 9-20, 2026.
Parents will receive an email with their child's results. A mental health team member from your child's school will contact you if there are areas of concern identified from the screening results.
The Iron County School District has received a Mental Health Screening Grant from the Utah State Board of Education, which covers the full cost of the well-being screener. In addition, support and resources may be available for students whose results indicate a higher level of need.
The Iron County School District has many supports and resources available to students and families. The district has access to community supports as well.
