What is PATH CPI?
California's Providing Access and Transforming Health (PATH) is a five-year, $1.85 billion initiative to build up the capacity and infrastructure of on-the-ground partners, such as community-based organizations (CBOs), public hospitals, county agencies, Medi-Cal Tribal and designees of Indian Health Programs, and others, to successfully participate in the Medi-Cal delivery system as California widely implements Enhanced Care Management (ECM) and Community Supports and Justice Involved services under CalAIM.
The PATH Collaborative Planning and Implementation initiative (CPI) provides funding for planning efforts to support the implementation of Enhanced Care Management (ECM) and Community Supports.
Who is Intrepid Ascent?
Intrepid Ascent is a consulting partner helping communities collaborate to deliver equitable whole person health. Since 2013, our expert teams have woven together strategic planning, governance, technology, quality improvement, and user-experience elements to match the multidimensional opportunities faced by our clients.
We are engaged as the facilitators for regional PATH CPI collaborative groups in two counties - Contra Costa and San Diego.
