How healthy communities work together

Intrepid Ascent is a consulting partner for community health. We engage with stakeholders from health care, government, social services, housing, schools, and other sectors to collaborate in serving shared populations. We do this by designing inclusive methods for decision-making, technology integration, and holistic client experiences that address social drivers of health and lead to greater equity.

We have been selected as the facilitators for the San Diego & Contra Costa PATH CPI collaboratives. As facilitators, we coordinate the different PATH CPI collaborative meetings, workgroups, and conversations each month; meet with participants to discuss their work with ECM and Community Supports; and elevate collaborative needs to support the improved implementation of CalAIM in the community.
