Contra Costa County PATH CPI

Welcome, Contra Costa!

Intrepid Ascent is responsible for hosting and facilitating monthly meetings and working with participants to develop programmatic activities contributing to CalAIM’s whole system, person-centered approach to care. CPI participants in Contra Costa County meet regularly to identify Enhanced Care Management (ECM) and Community Supports needs and gaps within the community, identify and resolve implementation issues, and monitor how Providing Access and Transforming Health (PATH) funds can be used to address implementation issues and disseminate best practices.

Contra Costa PATH CPI's meeting cadence is generally the fourth Friday of the month, from 9:00am to 10:30am PT. See the meeting calendar for up-to-date event information.

Contra Costa Collaborative 2023 Recap

2023 Aim Statement: By December 31, 2023, the Contra Costa County PATH CPI Collaborative will facilitate engagement between providers of Medi-Cal services and MCPs to improve Medi-Cal standards, systems, and processes which will increase the percentage of: ECM-eligible individuals enrolled in ECM services by 5% and CS-eligible individuals enrolled in CS services by 5%.

63 unique organizations serving Contra Costa County registered for the Contra Costa PATH CPI in 2023 and attended a variety of collaborative meetings offered each month. Attendees represented contracted Enhanced Care Management and Community Supports providers, non-contracted community-based organizations, local health systems, and the county's Medi-Cal Managed Care Providers. 

Over the entire year, the collaborative worked together to discuss a breadth of topics related to CalAIM and ECM/Community Supports implementation, including:  

Beginning in Q3, 2023, the collaborative began holding in-person meetings to support collaborative participants in building stronger networks with one another and to continue conversations that were started over Zoom. In our August in-person meeting, Intrepid facilitators conducted empathy mapping and ideal state brainstorming activities to help attendees place themselves in the shoes of their clients and ideate on the communities' highest priorities around ECM and Community Supports implementation. 

In November, Intrepid held two human-centered design sessions with stakeholders to answer the communities' identified highest priority: 'How might we (the community) use a geograhic map to better see Contra Costa County CalAIM service providers, contracted & non-contracted, and better understand their capacity so we can bet serve and make investments in our community together?' As the year wrapped up, Contra Costa PATH CPI participants began the first phase of a test-of-change to provide the data needed to build/visualize this geographic map of services and capacity.
