Thank you choosing to participation in the 2023 CFE program! We are so excited to work with you. 

We are conducting an evaluation study about the CFE program, and offer you the opportunity to consent for participation in that initiative. Your thoughts will inform our continued growth of the program and we hope you will approve! 

The evaluation is intended to measure the success of CFE program, as well as understand your experience in your schools more clearly. Our goal is not to evaluate your professional work or participation in the program. Research on this program is to uncover broad takeaways about the program as a whole. It is our hope that this study will provide insights that will help improve CFE in future iterations of the program, as well as help shed some light on the needs of teachers across the state. 

This form will ask you for your consent on two things: 1) Your participation in the CFE program, and 2) Your participation in the evaluation team's study of the CFE program. You can choose to participate in CFE (the program) while also declining to participate in the study.
