Student Acknowledgment and Commitment

I, _______________________________________ (Student Name), acknowledge that I have attended the physical education orientation meeting, reviewed the course policies, and understood the expectations for this class. I commit to upholding the following standards throughout the school year:

• Punctuality & Attendance: I understand that arriving up to 5 minutes late results in being marked late. Arriving 10 minutes or more late will result in being marked very late, with no exceptions or late passes accepted under any circumstance.

• Gym Access: I acknowledge that students may not enter the gym during lunchtime or any time where I do not have physical education class.

• Classroom Behavior & Safety:

  • I will use the bathroom log, writing my name and destination after receiving proper permission.

  • I understand that no food is permitted in the physical education environment; only water is allowed.

  • I will always adhere to the school uniform policy.

  • I understand that using any inappropriate language will result in being written up.

• Curriculum & Goals: I am aware that this course covers behavior, hallways, communication, and coursework.

• Grading Criteria: I understand that my grade will be calculated based on the following breakdown:

  • 60% Participation, 20% Fitness Test, 10% Preparation, 10% Effort

• Disciplinary Policy: I understand the behavioral escalation process for infractions, which consists of a warning, counselor involvement, parent contact, and a formal write-up/referral to the office.

PE Late Arrival Policies

Paper passes are not accepted. If you are late from another class:

• Same-Day Email Required: The teacher who kept you must email me your name and departure time before 11:59 PM today.

• Skyward Rules: Unexcused lateness (no email received) is logged directly into Skyward:

  • 5–10 Mins Late: Marked Late.

  • 10+ Mins Late: Marked Very Late (3 "Very Lates" = 1 Absence).

• Fixing Locked Records: Skyward locks at midnight. If your teacher forgets to email me today, you must have them email administration to adjust your record—I cannot change it after midnight.
  
  
  

Rule 2: Bathroom Log

• Sign In/Out Required: You must log your name, time out, time in, and reason (Bathroom Only) on the sheet by the gym door.

• One at a Time: If someone is already signed out, you must wait.

• No Unapproved Exits: Leaving without signing out, or leaving while someone else is out, will be reported as leaving class without permission.

• Forgot to Sign Out? Returning to class without having signed the log will result in a Very Late mark.

Rule 3: End-of-Class Dismissal

When class ends or the whistle blows, stop playing immediately and put away equipment. No "one more shot."

• 1st Offense: Verbal warning.

• 2nd Offense: Verbal warning.

• 3rd Offense: Behavioral Counselor referral + parent email.

• 4th Offense: Formal written write-up.
