Gan fod nifer y deietau arbennig y darperir ar eu cyfer ar gynnydd wrth i ni ddechrau gweithredu Prydau Ysgol am ddim i holl blant Ysgolion Cynradd, mae'n hollbwysig bod yr holl randdeiliaid yn ymwybodol o gyfrifoldebau allweddol er mwyn i ni sicrhau y gallwn ddarparu prydau addas/prydau wedi'u haddasu i'r disgyblion hynny sydd eu hangen yn unol â Rheoliadau Gwybodaeth am Fwyd 2014.

Oherwydd difrifoldeb rhai gofynion deietegol, argymhellir bod un ffurflen yncael ei dosbarthu i rieni/gwarcheidwaid i gasglu gwybodaeth o'r fath. Mae'r ffurflen hon wedi'i chynllunio fel y gellir ei chynnwys mewn pecynnau croeso ysgolion ac ati, ac fel y gellir ei rhoi i bob rhiant yn flynyddol i ganfod a oes unrhyw newidiadau i'r gofynion deietegol presennol. Efallai hoffech casglu'r wybodaeth ddiweddaraf yn flynyddol gan rieni/gwarcheidwaid yn electronig - gellir defnyddio'r templed hon.

Special Diets
Schools have a crucial role to play in relation to the management of data relating to pupils who require a special diet. Because the number of special diets being catered for is on the increase as we begin to implement Universal Primary Free School Meals (UPFSM), it is paramount that all stakeholders are aware of key responsibilities in order that we can ensure that we can provide suitably/adapted meals to those pupils that require them in accordance with Food Information Regulations 2014.

Special Diet Registration Form
Due to the severity of some dietary requirements, it is recommended that a single form is distributed to parents/guardians to capture such information. This form has been designed so that it can be included in school welcome packs etc. and so that it can be issued to all parents on an annual basis to ascertain if there are any changes to existing dietary requirements. You may wish to collect the annual updates from parents/guardians electronically - you can use this template.
This document has been designed to assist schools in identifying follow up actions that are needed for pupils that have dietary requirements, ensuring that the information is passed onto all relevant people.
The meeting template has been designed so that any discussions with parents/guardians can be documented and signed off by all parties so that an approved menu plan can be put in place.
