REQUIRED DOCUMENTATION NEEDED FOR REGISTRATION:
Proof of Residency – The items listed below are acceptable forms for proof of residency.
o Current utility bill - water, gas, or electric
o Deed of Trust
o Property tax statement
o Unexpired lease agreement showing the physical address
* If the proof of residency document is not in the parent/guardian’s name, then the homeowner on the proof of residency must print and complete the affidavit form, have it notarized, and return to your student's campus registrar. The parent and homeowner must be present at the campus when returning the completed notarized affidavit. The homeowner will need to provide identification to the registrar.
*A post office box address is not acceptable for proof of residency.
