Registration-Required Health Forms

Please fill out the following forms and upload them to your ParentVue account or bring them to your Student's School or the HPS District Office at 1515 West 8th St.

Along with the physical, vision, and dental forms we will also need a copy of the student's birth certificate and immunization record.
