Registration-Required Health Forms

Please fill out the following forms and upload them to your ParentVue account or bring them to your Student's School or the HPS District Office at 1515 West 8th St.

Nebraska Law requires a physical exam for all students entering kindergarten, seventh grade, and those transferring in from out-of-state. 

Along with the physical, vision, and dental forms we will also need a copy of the student's birth certificate, immunization record and custodial documents, if applicable.

• Kindergarteners:.   All children residing in the district who will reach the age of five (5) on or before July 31st of the calendar year are eligible to attend Kindergarten during the current school year.   All children residing in the district who are or will turn six years old before January 1 of the current school year are of mandatory attendance age.

• Students who are new to Hastings Public Schools, enterering Kindergarten, moving in from out of state or entering 7th Grade will also need exams completed between February and their start of the school year for the year they are enrolling or within 6 months prior to enrolling.

• Current Immunization records also need to be on file at each students school for all students.

• Sports physicals will be needed annually, prior to your student particpating in sports at Hastings Public Schools.

• An Over the Counter (OTC) Medication form is required if your student needs to take medication at school.

• After enrollment, schools may send home an Annual Health History form.
