Instructions

All students participating in the HSLJ Intramural Program are required to fill out the required Eagles Athletics Intramural Sports Participant Packet.

Instructions:

1. Download and print the Eagles Athletics Intramural Sports Participant Packet
2. Complete the Houston Independent School District Athletic Department Parents Approval for Participation in Athletics and Emergency Medical Authorization form. English and Spanish forms are provided.
3. Complete the Participation Physical Evaluation - Medical History Form. This form can be completed by parents.
4. Review the HSLJ Eagles Athletics Program Rules and Guidelines.
5. Complete the Intramural Sports Contact Information and Rules Acknowledgement form.
6. Complete the Intramural Sports Activity Fee and SchoolPay form as applicable.
7. Turn in the completed documents to Intramural Staff.
