A little about me...
I am a Hallettsville native, being born and raised here. My parents are Bill and Virginia Grafe. I married Rodney Haas and have four beautiful children. After high school I went to Victoria College and graduated from "THE" Texas A&M with a bachelors of science in Biomedical Science. I then went to PA school in Forth Worth at the University of North Texas Health Science Center with a degree in Physician Assistant studies. I love God, my family, sports, traveling, and reading!
Here at Hometown Wellness Clinic, I love our team of people who are willing to help others with a servant and caring heart. I love that we focus on wellness and prevention of diseases.

It's important to me that our community knows we really want to make a difference in their lives and their overall health. I am thankful for the opportunity to practice medicine focused on wellness and prevention. Hallettsville is a great community filled with wonderful hardworking people. I appreciate the trust and support this community has given to me throughout the years.
A little about me...
I am a hometown girl born and raised here in Hallettsville. I was one of those "Dornak girls" growing up. I knew I wanted to be in the medical field when I was in junior high. After graduating in 2005 from Hallettsville High School I obtained my Associates Degree in Nursing from Victoria College. After working for a few years I went back to obtain my Bachelor's of Science Degree from The University of Houston- Victoria. I went into Nursing Management and finally decided that I wanted to know the WHY behind an order that was given for me to carry out. I, again, went back to college to obtain my Master's of Science Degree in Nursing and became a Family Nurse Practitioner. I worked at Yoakum Family Practice for 10 years until life brought me a little closer to home. For the last year I have embraced the wellness aspect of primary care and truly enjoy continuing to provide the best care for my patients.
I married the love of my life Jeremy Haas in 2005 and together we have three beautiful children. Our world revolves around their academic and sports activities and we enjoy every second of the chaos.
Hometown Wellness Clinic gives off that feeling of being taken care of by a loved one. Whether it is my parent's friends, my own distant relatives or a brand new face the conversation always starts with "Now, who do you belong to?". When patient's can place who I am in their world they realize that I'm a true human being who is going to be real with them and not just write them off as another patient. Everyone has a connection and most patients always leave the clinic knowing they got the best care.
The services we offer here at HWC are unique. Again, I worked in a primary care office for 10 years. Had I known then what I know now about our services, I would have utilized them a lot more. CPAP, Hormone Evaluations, RPM, Cognitive/ Brain Health testing and the list goes on. Our CPAP program is top of the line and filled with professionals who know and understand not only the diagnosis, but the DME world that goes along with it. Our RPM program is probably my favorite! I adjust blood pressure medication daily. Having an open look at what my patient's readings look like daily is an asset to their care and how I can best manage it.
If I could shout from the rooftops to our community the one thing I'd like them to know about our clinic is simply that we care. We take pride in keeping our community healthy and giving them an open door where their needs are truly listened to. Any of the services we provide have one common denominator- they help keep our patients as healthy as they can possibly be. The patient only has to do their part!
A PCP (Primary Care Provider) is a medical professional who provides comprehensive and ongoing healthcare to individuals. They are often the first point of contact for health concerns and manage a wide range of common medical conditions, offer preventive care, and coordinate with specialists when needed.
An NP (Nurse Practitioner) is an advanced practice registered nurse who has completed advanced graduate education (master's or doctoral degree) and extensive clinical training. NPs are licensed to provide a broad range of healthcare services, including diagnosing and treating illnesses, prescribing medications, ordering and interpreting diagnostic tests, and providing health education and counseling. They often work independently or in collaboration with physicians.
A PA (Physician Assistant) is a healthcare professional who practices medicine under the supervision of a licensed physician. PAs are educated at the graduate level and trained to diagnose illnesses, develop and manage treatment plans, prescribe medications, and often assist in surgical procedures. They work in various medical specialties and settings, providing a wide range of patient care services.
