The Holy Name Faith Formation program caters to students in grades 1 through 9 and their families.

All families of program participants must first be registered members of Holy Name Parish.

There is a two year Sacramental preparation program for students wishing to receive First Reconciliation and First Communion or Confirmation.

Students in grade 2 and 9 regardless of where they attend school (Catholic, Private, Charter, Public) must register in the Faith Formation program as a part of the Sacramental preparation program requirements.

Registrations for the '23-'24 Faith Formation are to be completed electronically. All parents/guardians wishing to register their child in the program can do so via the page that corresponds to the child's grade level.
