BENEFITS SUMMARY:

Blue Cross Blue Shield of AL - Health, Dental, & Vision Insurance -

All employees are eligible for health, dental, and vision insurance within 30 days of employment. If you do not sign up by your 30 day eligibility, you will have to wait till open enrollment which is the month of May. Every May our premiums are reevaluated by BCBS & are subject to go up or down. Hereford Construction will pay 70% of the insurance premiums only leaving our employees responsible for 30% of the insurance premiums. The 30% of the premiums that the employees pay in will be deducted from paycheck every week as follows:

Employee Dental Insurance Costs per week:

• Employee - $2

• Employee & 1 or more kids - $6

• Employee & Spouse - $5

• Employee Plus (Family) - $9

Employee Health Insurance Costs per week:

• Employee - $38

• Employee & 1 or more kids - $70

• Employee & Spouse - $75

• Employee, Spouse, & Dependents(Family) - $107

Employee Vision Insurance Costs per week:

• Employee - $2

• Employee & 1 or more kids - $3

• Employee & Spouse - $3

• Employee Plus (Family) - $5

Company Match 401K through John Hancock. After being employed for 90 days you are eligible for a 401k contribution plan. Hereford Construction will match your contribution up to 3%. Your contribution will be taken out of your paycheck each week.
