Once your child's IEP meeting occurs, I will present you with a form, that is very similar to the one that is attached above, at the meeting for you to sign off on! It is Henry County's Medicaid/Teletherapy Consent form. This form covers two different things: Consent to bill Medicaid for services provided to your child and consent to provided teletherapy services (remote learning) to your child, given is is applicable.
