Hellgate Activities

Students who are interested in participating in any afterschool team sports at Hellgate Elementary must turn in three (3) completed forms:  

1) the Sports Physical Form completed by a physician and the parent/guardian 

2) the Parent Permission Form completed to allow permission to participate in each sport by the parent/guardian, and 

3) the Concussion Form completed by parent and student. Until all necessary forms are turned in to the school office, the student has not met all eligibility requirements to participate in that sport. All forms can be downloaded by clicking the link below. 

Your child may have to go through a Try-Out process, but all students that Try-Out are assigned a team. Schedules will be given to the student by their coaches after they have been assigned to a team. The coach will get the practice and game schedules to the players once finalized. 

The cost to participate is $75 per student per year. Students may participate in multiple activities but only pay the $75 activity fee once. Sports

All sports forms must be completed and submitted to the school office before student participation. 

All sports forms can be located on the Hellgate Activity web page. For your student to participate in Hellgate sports, we will need:

• a completed sports physical dated after May 1, 2026, completed by a physician

• a completed Parent Permission/ Medical Release form

• a completed Concussion form

• Signed Activity Handbook

• A one-time $75 Activity Fee

There are lots of places for signatures, initials, and information. Please review the forms to make sure all forms are completed before submitting them to the school office.  

INFINITE CAMPUS & Academic Eligibility

For a student-athlete to be eligible for athletics they may not have more than one F. If a student-athlete has more than one F they will sit out until their grades are brought up to no more than one F. You are always able to review your student's performance, grades, and access your student's grade progress via work submitted and see student work outstanding on Infinite Campus. If you have concerns regarding student performance and behavioral issues, please feel free to contact your student's teachers via email through IC. If you have not set up your parent portal for Infinite Campus, please contact the office. We are happy to assist you in using this helpful tool!
