Medication at School
Follow these guidelines:
o Medication Administration form filled out and signed by Prescribing Doctor.
o Parent Signature on same form
o Medication provided in original prescription bottle/box with label
o Ensure medication is not expired
o Pick up leftover medication at the end of the school year (any left will be destroyed per policy)
o A new form must be filled out by the Doctor every school year
o Parents must bring medication to clinic (do not send with student)
